Job Duties:- The APS Complex Case Coordinator is responsible for statewide oversight and coordination of APS complex case activities. This position supervises and directs the APS Complex Case Unit and ensures consistent, effective management of cases requiring specialized attention, including those involving the Office of Intellectual & Developmental Disabilities (OIDD) and the Office of Mental Health (OMH). The Coordinator monitors case progress, evaluates barriers to resolution, and provides guidance to staff regarding appropriate interventions.
The position leads and maintains collaborative partnerships with county APS offices, Adult Advocacy state office staff, and external agencies to support timely case movement, promote effective service delivery, and assist in achieving safe and appropriate outcomes for adults served. - Direct and manage the APS Complex Case unit. Monitor, coordinate and partner with OIDD (Office of Intellectual & Developmental Disabilities) & relevant agencies to ensure cases that require assistance from OIDD stay in the forefront and receive the services necessary for the adult to live as independently as possible. Collaborate and consult with the APS case management team, state office staff, and OIDD.
Carry case load as needed. - Monitor, coordinate and partner with OMH (Office of Mental Health) & relevant agencies to ensure cases that require assistance from OMH stay in the forefront and receive the services necessary for the adult to live as independently as possible. Collaborate and consult with the APS case management team, state office staff, local OMH offices and APS OMH consultant. Carry case load as needed.
- Research and complete a written summary on all cases beyond standard service duration. Consult with the case management team to guide and inquire about barriers to closing the case and ensuring all the adult’s needs are met before case closure. Carry case load as needed.
- Monitor, track, summarize, and identify trends on cases discussed in Case Reviews. Follow-up with the case management team (case managers and team leads) on recommendations made during Case Reviews and next steps. Summarize, monitor, and be able to communicate about any complex case inquired about by management staff.
- Assist APS State Office, County and regional staff by exploring long-term solutions, to include at times providing case management for adults in APS system with cases complex in nature. Provide regional coverage as needed.
- Perform other related duties as assigned.
A Bachelor’s Degree and three (3) years of professional experience in social services programs, correctional, education, business administration, general administrative management, or relevant program experience.
A Master’s Degree and one (1) year of professional experience in social services programs, correctional, education, business administration, general administrative management, or relevant program experience.
APS Experience Preferred.
Must possess a valid driver's license and have access to and be able to operate a motor vehicle. Individuals must comply with necessary field work assignments.
