South Carolina Department of Social Services is seeking a CWS Case Manager - Transition Youth Advocate to support youth aging out of foster care in the Extension of Foster Care (EFC) program. You will perform case management, service navigation, planning, and collaboration with legal and community resources to promote safety, permanency, and well-being.
Qualifications include a Bachelor's degree and two years of professional experience in social services. A Master’s Degree may substitute.
