Position: Extension of Foster Care Program Coordinator / 61061547

Job Duties:
  • The EFC Program Coordinator will implement the statewide Extension of Foster Care policy and practice in accordance with DSS Policy, Mission and GPS Practice Model. Regional EFC Team Coordinator position provides direct supervision to Child Welfare Regional Upstate and Pee Dee Team Leads in the Extension of Foster Care (EFC) Program under the Transition Services and Support Program of Child Welfare Operations in accordance with DSS Policy, Mission and GPS Practice Model.

    Aligns Regional work with Central office team with all resources, services, and supports for transition into adulthood. Supports ongoing CQI, training and professional development of staff being supervised. Monitors reports and compliance to ensure high alignment regarding transition age youth. Networks with and holds accountable regional providers serving transition age youth. Performs on-call duties and provides support to on-call staff.
  • Provides direct supervision of Regional Upstate and Pee Dee Team Leads. Provides leadership, supervision and support to team leaders within Extension of Foster Care by promoting adherence to DSS policies and procedures, agency mission, and GPS Practice Model. Assists the Program Manager with the development of plans and strategies to improve performance and practice.
  • Provides support, coaching, and oversight to the Team Leads. Promotes safety, permanency, and well-being by ensuring fidelity to policy and procedures, and conducting case consultations, facilitating meetings, reviewing data reports, monitoring policy changes, ensuring staff attendance in mandatory trainings, and conducting periodic case reviews to ensure quality practice. Provides direct supervision to case managers in absence of a supervisor. Coordinates and performs on-call duties.
  • Authentically engages and works collaboratively with child welfare staff, youth and young adults, and other professionals with empathy, respect and leadership to include timely responses to telephone calls, emails, and texts. Responds timely and professionally to constituent inquiries from the public and families to problem solve and bring resolution to inquiries.
  • Ensures and reviews policies and practices related to Extension of Foster Care are followed. Ensures that the agency is in compliance with legal requirements and court orders.
  • Provides supervisors with EPMS planning and evaluation plans. Participates in hiring and training of all new employees, promotes equal opportunity, develops a diverse workforce, and gives fair opportunity to all staff. Completes performance and practice evaluations on direct reports biannually and upon request for level ascension. Timely submits evaluations for level ascension and EPMS evaluations.
  • Maintains child welfare certification. Completes 20 hours of training yearly and adolescent development track. Ensures professional development for direct reports. Ensures staff attendance in mandatory trainings.
  • Ensures personal identifying information (PII) is properly used, accessed, gathered, shared and disposed. Protects the agency networks and applications by safeguarding systems, equipment and data.
  • Maintains an active routine of self-care and pursues support when necessary to minimize the threat of secondary trauma to physical and mental health. Understands the impact of trauma on families and secondary trauma on child welfare staff and applies principles to support families and staff.
  • Performs Red Cross shelter duty or other emergency shelter duties/services as directed.
  • Other duties as assigned.

A Bachelor's Degree in Social Work and four (4) years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health, and/or health care.

A Master's Degree in Social Work and two (2) years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health, and/or health care.

Child Welfare experience highly desired.

Must possess a valid driver's license and have access to and be able to operate a motor vehicle. Individuals must comply with necessary field work assignments.
