• Under the supervision of assigned licensed nurse, assists the hospice patient/ family with personal care, activities of daily living (ADLs) and other duties as instructed by the licensed nurse.
• Manages a patient case load and provides support to patient/families in the home/facility setting.
• Performs other job duties as assigned.

• High school diploma or equivalent.
• One year CNA, nurse aide, or home health aide preferred.
• Strong problem-solving, organizational, communication, and interpersonal skills.

• Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance.
• Frequent standing and walking for extended periods (8–12 hour per shift).
• Frequent bending, stooping, kneeling, crouching, and reaching.
• Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts).
• Visual acuity to read medication labels, physician orders, and monitors.
• Auditory ability to assess patient needs and respond to alarms or calls for assistance.
• Exposure to infectious diseases, blood borne pathogens, and bodily fluids.
• Frequent interaction with patients, families, and interdisciplinary team members.
• Fast-paced clinical settings with potential for high stress.
• Required use of personal protective equipment (PPE).
