Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on call or weekends)!

We are seeking a Full Time (30‑40 hours per week) provider for our Pocahontas Center located in Marlinton, WV. This position can be filled by a nurse practitioner or physician assistant. New grad NPs are welcome to apply. This role cannot support a new grad PA.

The base salary range for this position is $110,000 - $125,000 / year, in addition to our no‑cap incentive compensation bonus program.

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager and collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities. They also collaborate with the Medical Director and/or Attending Physician for patient‑specific medical care. The Provider offers primary and urgent care services to nursing center residents in collaboration with other medical providers and works collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving lives through high quality care and everyday compassion.

The Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner by completing an accredited Nurse Practitioner Program with a Master’s Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner. Other concentrations are considered based on experience and business need. The Physician Assistant must be a graduate from a program accredited by the ARC‑PA, pass the PA National Certifying Exam (PANCE), hold PA state licensure, and be NCCPA certified.

A minimum of two years of experience working with adult or geriatric populations is preferred. In states where practice is allowed, APRNs must have an unrestricted professional license and DEA; PAs must also have an unrestricted professional license and DEA for each state in which they practice.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification, job grade, location, and length of service. Eligibility for full‑time benefits requires a minimum of 30 hours per week. Certain benefits, including Health plans, are not available to part‑time employees working between 16 and 29 hours per week.
