Overview
Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on call or weekends)! We are seeking a Full Time (30‑40 hours per week) provider for our Pocahontas Center located in Marlinton, WV. This position can be filled by a nurse practitioner or physician assistant. New grad NPs are welcome to apply. This role cannot support a new grad PA.
The base salary range for this position is $110,000 – $125,000 per year, in addition to our no‑cap incentive compensation bonus program.

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. They collaborate on‑site with the Center Leadership Team for center‑specific goals and priorities, and with the Medical Director and/or Attending Physician for patient‑specific medical care.
The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with other medical providers in the center, working collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving the lives we touch through the delivery of high‑quality health care and everyday compassion.
Qualifications• Must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks may be considered based on experience and business need.
• Physician Assistants must be graduates of programs accredited by the ARC‑PA, have passed the PA National Certifying Exam (PANCE) for the PAC designation, possess PA state licensure, and be NCCPA certified.
• Minimum of two years’ experience working with adult or geriatric populations is preferred.
• In states where practice allows, APRNs must hold an unrestricted professional license and DEA for each state in which they practice; PAs likewise must hold an unrestricted professional license and DEA for each state in which they practice.
$110,000 – $125,000 per year.
