In-Home Nurse Practitioner or Physician Assistant Part Time - Morgantown

Overview

Signify Health, part of CVS Health®, is hiring a Part Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in‑home health evaluations. The role involves performing 45‑minute visits that include a medication and medical history review, a physical evaluation, and, if ordered by the member’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. The clinician uses an iPad and an intuitive clinical workflow and is not required to prescribe medicine or order lab tests.

• Conduct in‑home visits that connect one‑on‑one with patients.
• Review medication and medical history and provide a physical evaluation.
• Order diagnostic tests when authorized by the health plan (e.g., spirometry, diabetic eye exam).
• Use an electronic workflow on an iPad to complete documentation.
• Travel locally and within licensed states as required.
• Participate in virtual visits when needed.
• Deliver Diagnostic & Preventive Services, such as spirometry, DEE, and bone density screening.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required); multi‑state licenses (preferred).
• Experience as a Nurse Practitioner or Physician Assistant.
• Ability to safely perform duties in diverse home environments.

The primary workplace is the private residence of health plan members. Clinicians may encounter household pets, second‑hand smoke, or other potential allergens. Candidates must be comfortable performing duties in these settings.

Anticipated weekly hours: 29.5 (part‑time). Pay range: $37.66 – $81.11 per hour. Final base salary depends on experience, education, geography, and other factors.
