Job Title: Travel Registered Nurse (RN) – Home Health / Hospice
Employment Type: Travel Contract
Weekly Hour Guarantee: 40 hours
Work Schedule: Monday–Friday, 8:00 AM – 4:30 PM
On‑Call Requirement: Yes – every third weekend plus one additional day per week
Mileage Reimbursement: Yes (between patient visits, first patient to last patient)
Position OverviewWe are seeking a compassionate and experienced Travel Registered Nurse (RN) to provide high‑quality Home Health and Hospice care to patients within the assigned coverage area. This role focuses on delivering patient-centered care in the home setting, completing required OASIS assessments, and collaborating with interdisciplinary care teams to ensure positive patient outcomes.
Key ResponsibilitiesThe Registered Nurse will be responsible for the following day‑to‑day activities:
- Perform comprehensive OASIS assessments and follow‑up visits in accordance with regulatory and organizational requirements
- Deliver skilled nursing care in patient homes, including assessment, education, care planning, and documentation
- Accurately document patient visits, assessments, and outcomes in the electronic health record within required time frames
- Monitor patient conditions, identify changes, and communicate findings to physicians and care teams
- Educate patients and caregivers on disease processes, medications, safety, and care plans
- Coordinate care with interdisciplinary team members, including physicians, therapists, and social workers
- Adhere to productivity expectations of 40 points per week
- Participate in on‑call rotation as required
- Follow all clinical, safety, and infection‑control policies and procedures
- Current, unrestricted Registered Nurse (RN) license
- CPR certification (active and current)
- Ability to travel within the assigned coverage area
- Strong clinical assessment, documentation, and communication skills
- Previous Home Health or Hospice nursing experience
- Experience completing OASIS visits
- Ability to work independently in a field‑based environment
- Strong time‑management and organizational skills
- Registered Nurse (RN) license – required
- CPR certification – required
- Additional home health or hospice certifications are a plus
