Licensed Practical Nurse

Valley Center seeks a compassionate Licensed Practical Nurse to deliver hands-on nursing care in an on-site clinical setting. In this role, you will provide direct patient care, administer medications and treatments, monitor and document patient status, and support RNs and providers in executing care plans. You'll perform wound care, reinforce education for patients and families, and help maintain safety and infection control standards.

Ideal candidates are detail-oriented, comfortable using electronic health records, and committed to high-quality, respectful patient care.

Responsibilities

• Provide direct bedside nursing care under RN or provider direction
• Administer medications and treatments per physician orders and regulations
• Monitor, document, and report changes in patient condition using EHR systems
• Perform wound care, dressing changes, and basic procedures within LPN scope
• Support care planning and coordinate with interdisciplinary team members
• Educate patients and families on care plans, medications, and discharge instructions
• Maintain infection control, safety, and quality standards
• Assist with admissions, transfers, and discharges including documentation
