In-Home Health Assessment Nurse or Physician Assistant; PRN

Seeking a Nurse Practitioner to conduct comprehensive in-home health assessments for adult and geriatric patients.

Responsibilities Include

• Performing assessments covering medical history, diagnosis, treatment, and social factors
• Administering and ordering point-of-care and lab tests
• Collaborating with physicians, case managers, social workers, and caregivers
• Providing patient education and follow-up care
• Visiting members at their homes or facilities

• Master's Degree or equivalent, with NP licensure and prescriptive authority in the practice state
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care
• Current BLS, ACLS, or CPR certification
• Valid driver's license and automobile insurance
• Strong communication and computer skills; experience with medical records preferred
