Licensed Practical Nurse

Cedar Ridge Center, a Genesis Health Care skilled nursing and rehab facility in Sissonville, WV, seeks a Licensed Practical Nurse to provide person-centered care for short-term rehab and long-term residents. The LPN will deliver direct nursing care, administer medications and treatments, monitor resident conditions, and support rehabilitation goals. Working in a care-focused, team-oriented environment, you'll collaborate with RNs, therapists, and support staff to promote dignity, safety, and quality of life.

Responsibilities

• Provide direct nursing care to residents under RN and physician direction
• Administer medications and treatments per care plans and facility policies
• Monitor and document resident conditions, vitals, and responses to treatment
• Support rehabilitation and restorative care programs
• Collaborate with interdisciplinary team to develop and update care plans
• Educate residents and families on care, treatments, and health conditions
• Maintain infection control and safety standards
• Supervise and support CNAs and other support staff as assigned
• Report changes in resident status promptly to RN and physician
• Participate in quality improvement and compliance activities
