Job Summary

• Provide one‑on‑one skilled nursing care to pediatric and adult clients with chronic, complex, or medically fragile conditions in their homes.
• Collaborate with physicians, therapists, case managers, and family members to support the client’s individualized plan of care.
• Deliver continuous hourly care, ensuring safety, comfort, and clinical stability throughout each shift.

• Must have a valid Indiana nursing license in good standing.
• A minimum of one year of nursing experience as a Registered Nurse.
• Current CPR certification (Free re-certification with Adaptive).
• Good organizational and communication skills.
• Comply with state background screening requirements.
