The Primary Care Clinic (PCC) Social Worker assumes responsibility for the provision of social work services to veterans and their families receiving care at Salisbury VA Medical Center (SVAMC), one of its affiliated Health Care Centers or Community Based Outpatient Clinic (CBOC) primary care clinics and specialty clinics.
Major Duties- Conduct brief biopsychosocial assessments and provide single–session or short‑term, solution‑focused counseling to veterans and families; offer supportive counseling, case management, and referrals for resources and benefits.
- Establish effective therapeutic rapport with veterans/families referred by consultation, self‑referral, or treatment team members.
- Utilize assessments to facilitate veterans’ maximum use of treatments, aiming for the highest level of independence possible.
- Participate as a member of the interdisciplinary treatment team and collaborate on veteran‑centered treatment plans.
- Provide liaison services between veterans/families and VA and community resources, ensuring continuity of care and timely discharge planning.
- Implement and supervise advocacy, case management, and supportive group interventions; educate veterans and families on community resources, VA benefits, specialty programs, and advance directives.
- Maintain knowledge of veteran benefits, community resources, and referral processes; provide consultation to team members on psychosocial needs and their impact on treatment compliance.
- Maintain effective oral and written communication with diverse populations.
- Valid, accredited degree in social work (BSW, MSW, or equivalent). Transcripts and, if applicable, foreign equivalency certificates must be submitted.
- U.S. citizenship required;
Selective Service registration required for males born after 12/31/1959. - Proficiency in written and spoken English.
- Background/security investigation.
- Must be able to complete a trial period and possess appropriate identification (Real‑ driver’s license or state‑issued ).
- Participation in the seasonal influenza vaccination program is required for all VA Health Care Personnel.
