Job Title:
Clinical Medication Therapy Management (MTM) Pharmacist - NC Only
Work Setting: Remote, full-time position (40 hours per week) based in North Carolina, leveraging telehealth for service delivery.
Schedule: Weekday availability (Monday–Friday) with various shift times between 8:00 AM and 6:00 PM EST. (Schedules are provided two weeks in advance)
Collaboration: Work closely with pharmacy technicians and fellow pharmacists to ensure timely and documented services.
Core Function: Provide patient-centric Medication Therapy Management (MTM) services to optimize medication therapy, aiming to improve clinical, economic, and social outcomes.

Clinical Scope: Partner with physicians for comprehensive patient health optimization, including chronic disease state management and post-discharge follow-up care.
Essential Qualifications and Requirements:- Licensure: Active pharmacist licensure in the State of North Carolina (mandatory) and multi-state licensure preferred.
- Education: Pharm
D from an ACPE-accredited college or a graduate of a foreign college of pharmacy. - Certification: Completion of an MTM certificate course is required.
- Skills: Strong clinical therapeutic skills; demonstration of knowledge in appropriate medication dosing for adult and geriatric populations; ability to work independently.
- Technical: Competency in computer/technology use, a steady internet connection, and the ability to apply for a valid National Provider Identifier (NPI) Number.
- Experience: Prior pharmacy experience in a hospital or community setting is necessary.
- PGY1 Community Pharmacy or Hospital-based Residency Training.
- Board Certification through the Board of Pharmacy Specialties (BPS).
- Medication Therapy Management Certification (APhA) or Medication Reconciliation Certification (ASHP).
- Familiarity with EHR tools (Epic, Cerner, eCW).
- Experience using platforms like Outcomes MTM or Med Monitor.
A clinical enablement platform that partners with independent pharmacies across the U.S. to deliver reimbursable clinical services (RPM, CCM) to older adults with chronic conditions, utilizing AI and clinical workflows.
