Salary Range: $75,292.00 to $94,115.00 per year. This is an exempt (salaried) position. Pay rates are based on education, skill, experience level and internal equity. Clinical & Contractual Services

Provides technical assistance and consultation to Health & Human Services departments which includes behavioral health clinical consultation, nurse case management, and contract operations. CCS is responsible for providing guidance, knowledge, and expertise to each human services DSS division specific to medical and behavioral healthcare issues related to families, children, and adults served by DSS divisions.

Position Specific Information

This Nurse Case Manager position will work closely with Services For Adults (SFA) social workers, including Adult Protective Services, Guardianship, Community Social Workers, the Economic Services Division, and SOAR by conducting in‑home visits and medical records reviews for clients who have reports of neglect and/or abuse.

Assess client and client’s environment (in homes, shelters, residential facilities, other community settings) for ways to keep them safe as well as making recommendations for placement in higher level of care. Work with clients on ways to manage medications in the home.

Collaborate with attorneys, DSS personnel, and health care providers to ensure Adult’s needs are met.

Provide education to families and advocate for Adult’s well‑being and needs. Conduct medical record reviews, research and interpretation.

Position location is full‑time onsite at the Valerie

C. Woodard Center at 3205 Freedom Drive, Charlotte, NC 28208 and in the field as needed.

Position Summary

Works in a variety of settings coordinating care needs across the healthcare continuum. Incumbents ensure patients receive the most efficient and effective care for their particular condition, allowing for improved patient outcomes. Incumbents understand the system of care delivery for patients and communicate with doctors and specialists making sure services and treatments are delivered timely.

Essential Functions

Utilize professional training and the principles and practices of public health in the provision of health care services to all populations served.

Serve as a liaison/point of contact between the clinic and the patient/family for education, counseling, and general communication and coordination of care.

Provide ongoing support to clinical staff and external constituencies to ensure operational success.

Minimum Qualifications
  • Minimum of two years of nursing case‑management experience.
  • Bachelor’s degree or higher in Nursing from an accredited college.
  • Combination of relevant education and relevant experience accepted? No.
  • Licenses and

    Certifications:

    Current license to practice as a Registered Nurse (RN) in NC issued by the NC Board of Nursing or another Nurse Licensure Compact state;
    Certification in Basic Life Support for Healthcare Providers (CPR & AED) by the American Heart Association valid for at least 90 days after hire. Maintenance of this certification is required and may be supported as available. Requires a valid North Carolina or South Carolina Driver’s License, County Driving Privileges, and an Annual Motor Vehicle Record (MVR) check.
