Reports To: Clinical Team Leader
Status: Hourly
FT/PT/PRN: PRN
Location: Henderson County and surrounding areas
Position Summary:Providers nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.
- Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
- Serve as first responder for symptom-based calls from current patients after normal office hours
- Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly
- Participate in implementation and coordination of patient’s hospice care
- Assist with emergency admissions
- Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
- Record and report the results of the nursing assessment.
- Plan, initiate, deliver, and evaluate appropriate nursing acts.
- Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
- Collaborate with other health care providers in determining the appropriate patient and family-centered plan.
- Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
- Provide teaching and counseling about the patient's health.
- Report and record the plan for care, nursing care given, and the patient's response to that care.
- Supervising, teaching, and evaluating those who perform or are preparing to perform nursing functions and administering nursing programs and nursing services.
- Providing for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.
- Must be willing and able to care for patients with infectious disease processes unless approved accommodations
- Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
- Knowledge of HIPAA privacy rules and regulations
- Knowledge of electronic medical records systems
- Demonstrate excellent written and oral communication skills
- Ability to work under pressure to meet deadlines
- Ability to take initiative and utilize innovative techniques and ingenuity
- Ability to interact with various departments throughout the organization
- Ability to work flexible hours, if needed
- Ability to participate as a team player to coordinate projects
- Ability to plan, prioritize and coordinate multiple projects
- Demonstrate skill/expertise in pain and symptom management
- Demonstrate ability to effectively collaborate and facilitate the work of a group
- Demonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation
Experience:
- Two years of clinical experience preferred
Licensures &
Certifications:

- Valid NC RN license
- Valid Driver’s License and proof of automobile insurance
Physical Requirements:
- Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
- Flexion/Extension of arms/wrist
