Advocate Health is seeking a Home Health Registered Nurse to manage a patient caseload in the home care setting. You will document and deliver care per the plan of care and physician orders, coordinating with a multidisciplinary team to ensure quality outcomes.
You will work Mon–Fri 8:30am–5:00pm with on-call and weekend rotation, and may qualify for a sign-on bonus up to $15,000 and a referral bonus of $7,500 for internal teammates.
