Southern Health Partners Inc seeks a Licensed Practical Nurse to provide evening care Monday-Friday in a correctional facility setting. The LPN will perform assessments, administer medications and treatments, assist with chronic disease management, respond to sick calls and emergencies, and document care in EMR systems while ensuring compliance with facility and state regulations. You'll collaborate with multidisciplinary teams, support mental and physical health needs of incarcerated patients, and help maintain a safe, secure clinical environment.

• Provide direct nursing care to incarcerated patients during evening shifts
• Perform patient assessments and monitor vital signs
• Administer medications and treatments per provider orders
• Respond to sick call requests and urgent/emergent situations
• Document all care accurately in EMR systems
• Support chronic disease management and care plans
• Collaborate with physicians, RNs, and correctional staff
