The Hospice Liaison Nurse plays a critical role in connecting patients, families, and healthcare teams with compassionate end-of-life care services. As a trusted clinical resource, you will perform patient assessments, facilitate hospice admissions, and coordinate discharge planning to ensure a smooth transition from acute care settings to hospice services.

Working collaboratively with physicians, hospital case managers, social workers, healthcare partners, patients, and their families, you will advocate for patient-centered care while providing education and guidance on hospice eligibility, services, and care options. Your expertise will help ensure timely access to hospice care, improve patient outcomes, and support individuals and families during a significant stage of their healthcare journey.

• Conduct comprehensive clinical assessments to evaluate hospice eligibility and patient needs.
• Facilitate hospice admissions and coordinate seamless transitions from hospitals and other healthcare settings.
• Collaborate with physicians, case managers, discharge planners, and interdisciplinary healthcare teams to develop appropriate care plans.
• Educate patients, families, and referral sources on hospice philosophy, services, and benefits.
• Build and maintain strong partnerships with hospital staff and community healthcare providers.
• Support discharge planning efforts and help remove barriers to timely hospice placement.
• Ensure compliance with regulatory requirements, organizational policies, and clinical standards.
• Serve as a compassionate advocate for patients and families, promoting dignity, comfort, and quality of life.

This position is primarily hospital-based, serving as the on-site clinical liaison between healthcare providers and hospice services. Occasional field-based admissions and patient visits may be required based on operational needs.

• Education:
  
  High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred. Graduate of an accredited school of nursing required; BSN preferred.
• Experience:
  
  Minimum three years' experience in home care or hospice, required. Previous experience in managed care, infusion care, and case management and previous home health/hospice case management experience, preferred.
• Licensure/Certification:
  Current RN licensure in appropriate state, required.
• Additional Skills (required):
  • Ability to provide direct patient care, demonstrate excellent nursing skills
  • Excellent verbal and written communication skills, problem solving skills, and basic math skills.
  • Understands hospice philosophy and issues of death/dying and demonstrates competency in pain and symptom management with EOL care.
  • Ability to interpret information to identify each patient’s requirements for care relative to his/her age specific needs.
  • Ability to prioritize, organize and handle varying and unpredictable situation such as emergencies or crisis situations.
  • Must be flexible, versatile, adaptable to change, with good critical thinking, problem solving and diplomacy skills.
  • Knowledge of procedures and nursing standards of practice, both general and specific to the specialties of medical- surgical nursing.
  • Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees.
  • Actively identifies new areas for learning; regularly creating and taking advantage of learning opportunities; using newly gained knowledge and skill on the job and learning through their application.
  • Must have computer experience and have reliable transportation to and from all work related assignments and activities.
  • Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.
  • Must be available for after hours call on rotational basis.
  • Specific vision abilities required by this job include close vision, distance vision, night driving, and ability to adjust and focus.
