Job Title
Assumes primary responsibility for direct case coordination services for participant self-sufficiency and self-reliance; ensures compliance with all relevant contractual requirements, as well as agency policy and procedures; as assigned, directs the work of staff/volunteers in provision of participant services. This position is limited duration and intended to support short-term needs, with the duration subject to change based on organizational needs.
Essential Functions- Provides on-going case coordination; conducts intake interviews and assesses needs of participants; develops service plans.
- Provides advocacy, support, and connection to social services agencies and to appropriate employment, advising and assisting participants or making appropriate referrals.
- Establishes and maintains case files; performs clerical duties associated with case coordination, including correspondence and reports. Prepares and maintains required documentation and requirements of the agency's CMIS and State Refugee Office's RIS data base systems.
- As appropriate, conducts home visits to address health, safety, and job preparedness issues; transports participants, or directs such activity, as necessary to ensure provision of services.
- Maintains working relationships with staff, other diocesan and external resources to coordinate participant services and provide advocacy as needed; advises Supervisor of activities to ensure that services offered are in keeping with the contract requirements.
- Represents the refugee program and the Agency in the community; responds to inquiries.
- Participates in EOS and supports Agency compliance with relevant accreditation standards.
- Participates in outreach and development activities as needed.
- Participates in program, agency, diocese and stakeholder meetings and training as required.
- Performs other duties as required, in collaboration with Supervisor and other program staff.
Education, Experience and
Skills Required:
- Graduation from high school or equivalent.
- Experience with diverse cultures.
- Continuous possession of a valid driver's license; continuous status as an Approved Driver by the Catholic Diocese of Charlotte; continuous insurance coverage meeting requirements of the Catholic Diocese of Charlotte on any personal vehicles used as part of employment.
- Fluency in foreign language of program participants preferred but not required.
- Skills required:
Work with discretion, and communicate effectively with staff, volunteers, clientele and the public in a culturally diverse working environment; prepare and maintain records according to contract and agency standards; communicate fluently, orally and in writing, in English and specific native language as applicable, to perform the essential functions this position.
Working Environment:
Responsibilities of the Case Aide may involve travel and work beyond the regularly scheduled workday, and will involve direct interactions with program participants, including in the participants' home, agency/personal vehicle or various public settings.
