The Case Manager assesses participant needs, identifies essential resources, and coordinates individualized service treatment plans. Working within a holistic, values-based framework, this role serves as an advocate for participants, builds community support networks, and connects individuals with critical services in behavioral health, addiction recovery, or IDD settings.
Key Responsibilities- Team-Based Service Planning:
Collaborate with a multidisciplinary team (clinical, medical, and external partners) to develop, review, and execute individualized recovery plans. - Direct Client Care & Community Outreach:
Deliver mobile, community-based support to help participants access resources, increase community integration, and achieve personal recovery goals. - Resource Development & Advocacy:
Establish relationships with community agencies, identify service gaps, and advocate on behalf of participants. - Documentation & Compliance:
Complete all required assessments, progress notes, and administrative records accurately and in compliance with agency, funder, and licensing standards. - Coverage:
Provide coverage and support as needed to maintain required 24-hour access to Intensive Case Management services.
- Education:
Bachelor's degree in Human Services or a related field. - Experience:
Minimum of 2 years providing direct support in Behavioral Health, Addiction Recovery, or IDD services; 1 year of Case Management experience preferred. - Technical & Licensing Requirements:
Valid driver’s license and intermediate computer skills.
Physical Requirements
- Field Work:
Primarily community-based within the program’s service delivery area; requires driving participants to appointments. - Physical Effort:
Medium work—frequently exerting up to 20 lbs of force and occasionally up to 50 lbs to lift, carry, push, pull, or move objects. - Mobility & Communication:
Frequent sitting, standing, walking, bending, reaching, and active verbal/written communication.
