Overview

The Genitourinary Reconstructive Advanced Practice Provider (APP) is an integral member of an academic urology team specializing in the care of patients with complex pelvic floor and lower urinary tract disorders. This role focuses on the evaluation and management of patients with pelvic floor disorders, urinary incontinence, overactive bladder, urinary retention, pelvic organ prolapse, genitourinary fistula, voiding dysfunction, urinary tract infections, neurogenic bladder, and complications related to prior pelvic or urologic surgery.

The position is structured with initially supervised clinical practice and a defined progression to independent practice, based on demonstrated competency, institutional privileging, and departmental approval. The APP practices in close collaboration with fellowship-trained faculty in endourology, genitourinary reconstructive surgery, and urogynecologic pelvic surgery within a tertiary and qua ternary academic referral center.

Administration
  • Documents clinical encounters in the electronic health record in a timely and accurate manner
  • Coordinates care with internal and external providers, schools, and community agencies

  • Assists with care coordination, referrals, and follow up planning for patients with complex needs

Clinical
  • Evaluate and manage patients with pelvic floor disorders, including pelvic organ prolapse, stress urinary incontinence, urgency urinary incontinence, mixed incontinence, and overactive bladder. This will include performance and interpretation of urodynamic studies, flow rate and post void residuals, bladder diaries as well as other diagnostic testing related to these specialty diagnoses.
  • Provide comprehensive care for patients with genitourinary fistula, urethral stricture disease, urinary retention, voiding dysfunction, neurogenic bladder, recurrent urinary tract infections and post-surgical complications
  • Obtain detailed urologic, gynecologic, obstetric (when applicable), sexual, and surgical histories
  • Perform focused genitourinary and pelvic examinations, including prolapse assessment
  • Develop and implement individualized, evidence-based treatment plans in collaboration with attending physicians
  • Provide longitudinal management of chronic pelvic and lower urinary tract conditions
  • Interpret and apply diagnostic studies including urodynamics, cystoscopy findings, imaging, and laboratory results
  • Coordinate multidisciplinary care with pelvic floor physical therapy, urogynecology, gynecology, colorectal surgery, transplant, nursing, and referring providers
  • Provide pre-operative counseling, perioperative coordination, and post-operative follow-up care
  • The APP will receive supervised training with progression to independent performance, based on competency and credentialing, in the following outpatient procedures commonly performed in pelvic and reconstructive urology:
  • Flexible cystoscopy
  • Urethral dilation and difficult catheter placement
  • Foley catheter insertion, exchange, and troubleshooting
  • Suprapubic tube exchanges and management
  • Voiding trials and post-void residual assessment
  • Urodynamic testing, including performance and interpretation
  • Assistance with and interpretation of retrograde urethrograms (RUG) and voiding cystourethrograms (VCUG), nephrostograms, looporams and other related studies when applicable
  • Intravesical instillations, including botulinum toxin for overactive bladder (when credentialed)
  • Pessary evaluation, fitting, counseling, and management (if included in institutional scope)
  • Minor postoperative wound care and vaginal, perineal, or genital assessments
  • Management of urinary diversion appliances and stoma care, when applicable
