Overview
A transformational leader responsible for supporting the Rehabilitation Services Manager and Home Health Director in the day‑to‑day operations of the Home Health Rehabilitation Services Department. Responsibilities include effective resource utilization, program and policy development, implementation and evaluation. As a member of the Rehabilitation Services leadership team, the individual demonstrates clinical expertise and will be involved in continuous quality improvement, customer service, resource and financial management, efficient and effective system processes and professional development.
Will perform patient care as needed.
At UVA Continuum Home Health Care, our therapists provide care to individuals in their homes. Home healthcare clinicians are responsible for assessing, identifying, planning, and providing care with each patient in the home environment. Our home health therapists work collaboratively to develop and implement a patient‑centric plan of care for each individual, and they provide care as part of regional interdisciplinary teams.
Our clinicians work closely with community health providers, UVA Health, and others to ensure safe delivery of care in the home.
UVA Continuum Home Health Care offers therapy clinicians autonomy in their professional practice, scheduling support (including self‑scheduling), mileage reimbursement at the federal level, and a clinical ladder for career advancement. We do not require candidates to have previous home care or OASIS experience. We provide an extensive onboarding orientation/training program for all new therapy clinicians who join our team.
UVA Continuum Home Health Care is the Home Health department of UVA Health. We are an academic home health agency, providing care in the community for almost 30 years. We serve Central Virginia, including Charlottesville, Albemarle, Augusta, Rockingham, Harrisonburg, Staunton, Waynesboro, Nelson, Buckingham, Fluvanna, Louisa, Orange, Madison, Greene and Culpeper. UVA Health is Magnet recognized, Medicare certified, and accredited by the Joint Commission.
Job Responsibilities- Assists Manager or Director in ensuring the highest and safest level of care is received by all patients served according to professional standards, regulatory and departmental guidelines.
- Serves as an integral member of the Rehabilitation Services leadership group.
- Shares accountability with Manager or Director for unit/department/organizational goals achievement including patient experience, team member engagement and continuous process improvement.
- Will assist Manager or Director with recruitment, retention, coaching, training, evaluation and development, as well as preparation and delegation of work schedules and workflow.
- With supervision from Manager or Director, participates in basic human resource functions.
- Provides managerial support as required. Develops, implements and participates in Quality Improvement Processes activities. Works with members of the health care team in activities designed to improve patient care outcomes.
- Provides leadership and serves as a resource in the provision of patient care and department activities. Assists assigned staff in development of problem solving and decision‑making skills.
- Maintains all records pertaining to the staff competency and quality audits and other related records of assigned staff.
- Serves as a professional role model, coach, and resource for employees. Participates actively in department and/or hospital committees and work groups.
- Assists Manager or Director in fiscal management of department including managing of supplies, labor, budget preparation and benchmarking.
- Performs therapeutic assessment and treatment according to established guidelines.
- Performs other related duties as assigned.
Physical Therapy
Education:
Minimum – Bachelor’s Degree in Physical Therapy;
Prefer Master’s or Doctoral Degree.
Experience:
Must have acute care hospital experience; home health experience preferred strongly. Prefer 3 years of demonstrated leadership experience.
Licensure:
