Seven Healthcare is pleased to present a Travel Case Management RN opportunity in Charlottesville, Virginia, offering a weekly blended rate of $2,500 based on five 8‑hour day shifts. This role is ideal for an RN with strong clinical judgement, discharge‑planning expertise, and the ability to coordinate complex patient needs across the continuum of care.

• Conduct initial and ongoing discharge assessments to identify patient needs and care barriers
• Develop individualized discharge plans, coordinating with physicians, therapy, social services, and outside providers
• Support transitions to home health, skilled nursing, rehab, outpatient services, or community‑based programs
• Perform utilization review and collaborate with insurance representatives as needed
• Document all case‑management activities accurately and on time
• Educate patients and families regarding post‑acute care, community resources, and safety
• Promote continuity of care and prevent avoidable readmissions

• Active Virginia RN license or compact eligibility
• Current BLS certification
• Minimum 1–2 years' recent Case Management or Care Coordination experience
• Strong communication, assessment, and organizational skills
