** Float Clinical Pharmacist
** to provide clinical and operational support across multiple inpatient service lines, helping ensure safe, effective, and high-quality medication use for our patients. In this role, you will apply
** your broad acute care knowledge
** while developing deeper familiarity with a wide range of specialties and care environments. On a day-to-day basis, you will: + Provide clinical pharmacy services across a
** variety of acute care and inpatient practice areas
** based on operational and patient care needs + Review, verify, and manage medication orders with a s
** trong emphasis on patient safety** ,evidence based practice, and therapeutic optimization + Serve as a clinical resource to physicians, advanced practice providers, nurses, and other members of the care team + Identify, prevent, and resolvemedication related problems for patients with complex and evolving clinical conditions + Support transitions of care through medication reconciliation, monitoring, and patient specificrecommendations + Contribute to
