Job Duties

The Blue Ridge Health District (BRHD) is seeking a passionate Population Health Coordinator to join our team of dedicated public health professionals. BRHD serves the counties of Albemarle, Fluvanna, Greene, Louisa, Nelson, and the City of Charlottesville.

The Population Health Coordinator serves as a key member of the Population Health Division, supporting community engagement, communications, and coordination of public health initiatives. This position plays a central role in building relationships with community partners, developing public-facing educational materials, and supporting major population health efforts across the District.

What you’ll do:
  • Coordinate community engagement activities for the Population Health Division.
  • Support communications efforts by writing newsletters, managing social media content, and developing public information materials.
  • Manage BRHD's Canva account and be responsible for creating graphics, flyers, social media posts, educational materials, and other visual content.
  • Work closely with Population Health Division staff to support the MAPP2

    Health Community Health Assessment and Community Health Improvement Plan (CHA/CHIP).
  • Assist with projects involving Community Health Workers, WIC, maternal and child health, and other public health programs across the District.
  • Build and maintain relationships with community partners, including coordinating meetings, events, outreach, and collaborative projects.
  • Help gather, organize, and share information that supports population health strategies and community well-being.
  • Provide general coordination support to ensure smooth operation of Population Health Division initiatives.

Minimum Qualifications
  • Knowledge of Health Education & Promotion:
    Understanding of population health data collection, community demographics, social/cultural environments, and chronic disease prevention. Ability to implement and evaluate health improvement programs and policies for diverse populations.
  • Communication

    Skills:

    Excellent oral and written communication skills to effectively guide staff, educate internal/external stakeholders, and respond to data requests. Ability to develop and deliver training materials, seminars, and health education sessions.
  • Resource Management:
    Knowledge of procurement processes, budget monitoring, and ensuring proper use of resources. Ability to prepare grant applications, manage grant activities, and maintain compliance with funding guidelines.
  • Collaboration & Relationship Building:
    Ability to build and maintain effective working relationships with community partners, local health districts, and agencies to promote health initiatives and improve population health.
  • Quality Improvement & Data Analysis:
    Skilled in analyzing and verifying data, identifying trends, and preparing reports to support health program planning. Ability to evaluate the success of health programs and make necessary adjustments.
  • Technology

    Skills:

    Proficiency in using social media platforms for health communication, as well as experience with data management systems and office software (Word, Excel, PowerPoint).
  • Project Management:
    Ability to develop and manage project plans, timelines, and resources to ensure successful implementation of health initiatives
Additional Considerations
  • Experience conducting CHA/CHIP activities
  • Proficiency with Canva or similar graphic design platforms
