Overview
Practice Nursing with Purpose and Balance. Located in the heart of Charlottesville, VA, this on-site role allows you to support a dedicated patient population while working alongside a collaborative, interdisciplinary care team. As an RN Care Coordinator at CareATC, you’ll serve as a central clinical resource for patients managing chronic conditions—providing care coordination, patient education, and proactive outreach to support prevention, treatment adherence, and long-term health outcomes.
This role is ideal for nurses who want to step away from traditional bedside or volume-driven care and focus instead on clinical assessment, care planning, and relationship-based support. You’ll have the time and structure to build trust with patients, coordinate care thoughtfully across providers and services, and practice nursing in a way that feels sustainable, balanced, and meaningful.

- Annual Salary Range: $82,000-$90,000 ( Based on a 40-hour work week. Compensation is determined by experience and credentials.)
- Conduct clinical assessments and ongoing patient outreach
- Support chronic disease management and care coordination programs
- Educate and empower patients to self-manage their health
- Coordinate care across providers, clinics, and services
- Document care plans and patient interactions in the EMR
- Participate in interdisciplinary team meetings
- Bachelor’s degree in nursing
- Active RN license in Virginia. Compact License strongly preferred.
- 3–5 years of clinical or care coordination experience
- Strong assessment, communication, and organizational skills
- A patient-centered, collaborative approach to care
