Job Summary
The RN Case Manager is responsible for providing comprehensive case management services to high-risk members. This role focuses on care coordination, utilization management, and patient advocacy to ensure cost-effective, high-quality outcomes across the continuum of care.
Primary Responsibilities
- Perform comprehensive assessments of high-risk members to identify clinical, functional, and psychosocial needs
- Collaborate with primary care providers to develop and implement individualized, comprehensive care plans
- Identify, recommend, and continuously reassess appropriate and cost-effective levels of care
- Coordinate care transitions and collaborate with physicians, social workers, discharge planners, and claims professionals
- Arrange non-medical supports (e.g., housing, transportation) to promote compliance with treatment plans
- Utilize specialty resources as needed to support optimal patient outcomes
- Maintain accurate and detailed documentation of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Proactively prevent adverse patient outcomes and intervene promptly when issues arise
- Advocate for patients while ensuring privacy and confidentiality standards are upheld
- Associate's Degree in Nursing or related field required
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Case Management certification preferred or required
- Strong ability to analyze complex medical information and make sound clinical decisions
- Experience with Homecare Homebase required
- Excellent communication, organizational, and care coordination skills
- Thursday through Sunday
- Pay: $53 an hour with weekly paychecks!!
