Registered Nurse - Case Management - Hospital

Job Description

Job Summary:

Seeking a skilled Registered Nurse with hospital case management experience to lead inpatient care coordination. This 13-week assignment involves day shifts, with occasional weekends and holidays, requiring acute care hospital inpatient experience and Epic proficiency.

Responsibilities:

• Manage inpatient case loads, develop care plans, and coordinate discharge planning.
• Rely on Epic EMR to document patient interactions and care progression.
• Approve up to 40 hours of RTO, ensuring requests are accurate at submission.
• Maintain compliance with travel and license requirements, including pending licenses in hand before start.
• Float within scope of practice as needed across various units within the facility.
• Adhere to facility policies regarding background checks, certifications, and module completion.

Qualifications:

• Possess at least 13 weeks of hospital inpatient case management experience in an acute care setting.
• Hold a valid state RN license, with RRT travelers having license in-hand at submission.
• Demonstrate proficiency with Epic EMR systems.
• BSN degree preferred, Nursys verification required.
• Ability to work day shifts (8:00-16:00) and adhere to travel compliance policies.
• Ability to communicate effectively and coordinate care across multidisciplinary teams.
