Your expertise in advanced heart failure management can directly shape outcomes for one of the most complex and high acuity patient populations in cardiovascular medicine. In this role, you will serve as a key clinical partner within a multidisciplinary heart failure team, contributing to the comprehensive management of patients across the inpatient setting. This is a fast paced, high‑acuity environment where critical thinking, strong clinical judgment, and effective communication are essential.

A typical day includes both independent clinical decision making and close collaboration with physicians and interdisciplinary team members to ensure optimal patient outcomes.

You will:

• Provide direct inpatient care for patients with advanced heart failure, including those requiring complex therapies and interventions
• Conduct daily rounding, detailed patient assessments, and real-time care plan adjustments based on evolving clinical needs
• Collaborate closely with cardiologists, intensivists, surgeons, nursing staff, and fellow APPs to coordinate and execute care plans
• Support admissions, discharges, and transitions of care

• Deliver patient and family education
