As an RN Inpatient Case Manager at UVA Health, you will combine your bedside knowledge with care coordination expertise to guide patients through complex healthcare journeys. This role offers a highly collaborative environment where your clinical judgment directly impacts discharge planning, patient flow, and the overall quality of care.
The Role:As an Inpatient RN Case Manager, you serve as a key connector across the care team, ensuring that each patient's clinical needs align with a safe and effective discharge plan. Your work requires strong clinical insight, communication, and the ability to navigate complex systems and priorities.
In this role, you will:
- Complete initial assessments on all newly admitted patients
, identifying clinical needs and potential discharge barriers - Participate actively in interdisciplinary rounds
, contributing to real-time care planning and decision-making - Coordinate post‑acute care referrals
, including rehabilitation, home health, and community‑based services - Collaborate closely with physicians, nursing teams, and ancillary staff to align treatment plans with discharge strategies
- Document care coordination efforts accurately within EPIC to support continuity and compliance
This is a dynamic, fast‑paced role where you will manage multiple patients across varying levels of acuity. Your ability to anticipate needs, prioritize effectively, and communicate across disciplines is critical to success.
Schedule:- Monday through Friday, 8:00/8:30 AM - 4:00/4:30 PM
- Every 4th weekend rotation, with days off provided before and after the weekend

Minimum RequirementsEducation:
Graduate of accredited nursing program. Bachelor's of Science in Nursing degree required within 5 years of hire or transfer into Case Management role.
Experience:
2 years of relevant experience required.
Licensure:
Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required.
Job requires standing for prolonged periods, frequently walking, bending/stooping. Proficient communicative, auditory and visual skills;
Attention to detail and ability to write legibly;
Ability to lift/push/pull 20-50 lbs. May be exposed to chemicals, blood/body fluids, and infectious disease.
