Our Home Health LPNs provide expert, patient‑centered care.

Office

Location:

Charlottesville, VA

Coverage area: South/West Territory, Waynesboro to Buckingham

Schedule: Full‑time;
Monday ‑ Friday

Home Health and HCHB experience preferred.

• Provide 1:1 care for clients following the client's plan of care at all times while maintaining dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed per the client's plan of care and assessments of results.
• Accurately document nursing actions, communication with family, case manager, and physicians, and track progress and outcomes for established goals, informing stakeholders of changes in medical condition and needs.
• Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are available.
• Ensure all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.
