Your expertise in advanced heart failure management can directly shape outcomes for one of the most complex and high acuity patient populations in cardiovascular medicine. As an Advanced Practice Provider on the Inpatient Advanced Heart Failure team, you will play a central role in delivering highly specialized care to patients requiring complex cardiac management, supporting both their immediate needs and long-term treatment trajectories.

* In this role, you will serve as a key clinical partner within a multidisciplinary heart failure team, contributing to the comprehensive management of patients across the inpatient setting. This is a fast paced, high‑acuity environment where critical thinking, strong clinical judgment, and effective communication are essential.

A typical day includes both independent clinical decision making and close collaboration with physicians and interdisciplinary team members to ensure optimal patient outcomes.

You will:

+ Provide  
** direct inpatient care
** for patients with advanced heart failure, including those requiring complex therapies and interventions

+ Conduct  
** daily rounding, detailed patient assessments, and real-time care plan adjustments
** based on evolving clinical needs

+ Collaborate closely with  
** cardiologists, intensivists, surgeons, nursing staff, and fellow APPs
** to coordinate and execute care plans

+ Support  
** admissions, discharges, and transitions of care** , ensuring continuity and patient safety

+ Deliver  
** patient and family education** , helping individuals better understand diagnoses, treatment options, and long-term disease management

+ Contribute to a  
** high-functioning, team based model of care
** focused on clinical excellence, efficiency, and compassionate delivery
