As an Advanced Practice Provider on the Inpatient Advanced Heart Failure team, you will play a central role in delivering highly specialized care to patients requiring complex cardiac management, supporting both their immediate needs and long-term treatment trajectories.

The Role:

In this role, you will serve as a key clinical partner within a multidisciplinary heart failure team, contributing to the comprehensive management of patients across the inpatient setting. This is a fast paced, high‑acuity environment where critical thinking, strong clinical judgment, and effective communication are essential.

A typical day includes both independent clinical decision making and close collaboration with physicians and interdisciplinary team members to ensure optimal patient outcomes.

You will:

* Provide direct inpatient care for patients with advanced heart failure, including those requiring complex therapies and interventions

* Conduct daily rounding, detailed patient assessments, and real-time care plan adjustments based on evolving clinical needs

* Collaborate closely with cardiologists, intensivists, surgeons, nursing staff, and fellow APPs to coordinate and execute care plans

* Support admissions, discharges, and transitions of care, ensuring continuity and patient safety

* Deliver patient and family education, helping individuals better understand diagnoses, treatment options, and long-term disease management

* Contribute to a high-functioning, team based model of care focused on clinical excellence, efficiency, and compassionate delivery
