Position: Registered Nurse Care Coordinator (RNCC) - UMA Clinic

Overview

These roles involve patient care, including assessment, diagnosis, treatment, and follow‑up. The positions may be performed across various healthcare settings such as hospitals, clinics, long‑term care facilities, and community health organizations.

Key Responsibilities
  • Provide medical care, including assessment, diagnosis, treatment, and follow‑up.
  • Support patients and their families, ensuring effective teamwork.
  • Guide patients through the healthcare system and coordinate care across multiple providers.
  • Diagnose, treat, and care for patients within a clinical discipline or specialty.
  • Develop and implement clinical programs, policies, and services using specialized knowledge.
  • Perform care navigation, ensuring timely access to resources and support throughout the treatment journey.
  • Communicate necessary information requiring explanation or interpretation.
Minimum Requirements
  • Education: Bachelor of Science in Nursing from an accredited nursing program.
  • Experience: 1 year of registered nursing experience required; 3 years of related and relevant registered nursing experience strongly preferred.
  • License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.
  • Specialty: Specialty board certification strongly preferred.
Physical Demands

Job requires standing for prolonged periods, frequent traveling, and bending/stooping. Must have proficient communicative, auditory, and visual skills; attention to detail; ability to write legibly; ability to lift/push/pull 20–50 lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.

Pay Range

The pay range for this role is $85,820.80 – $ annually. Individual compensation will be determined by the selected candidate’s qualifications, previous work experience, and/or education.
