Registered Nurse Care Coordinator; RNCC - Pediatric Endocrinology and Diabetes, Children's Hos

The Registered Nurse Care Coordinator (RNCC) is responsible for (deliberate) organization of patient and family-centered care activities between two or more participants in which the RNCC uses advanced critical thinking to develop, implement, and evaluate individualized care plan to empower patients and optimize outcomes. The RNCC marshals and coordinates resources and patient care activities to support transition management, through collaboration across the continuum, supporting patient and families’ needs for individualized follow-up and coordination of services across healthcare services and locations.

As a Registered Nurse Care Coordinator in Pediatric Endocrinology and Diabetes , you will blend ambulatory nursing practice, patient education, and care coordination to support children with Type 1 and Type 2 diabetes and other endocrine disorders. • Coordinate care for pediatric patients with diabetes and endocrine disorders across clinic and hospital settings
• Provide in-depth patient and family education

  • Insulin pump training and ongoing disease management support
  • Teaching in both classroom-style settings and at the bedside for admitted patients
• Manage phone triage and patient communication

• Collaborate closely with physicians, advanced practice providers, and fellow RN Care Coordinators in a highly integrated team model
• Participate in a rotating Saturday call schedule shared among the RNCC team
• Progress toward independent teaching, clinical decision-making, and triage management within the first year

This role is primarily clinic-based and in-person
, offering consistency in schedule while maintaining meaningful patient interaction throughout the day. The position is part-time, with a preference for candidates available closer to 24 hours per week.
