We are currently seeking an experienced and highly skilled Registered Nurse
- Liaison to join our interdisciplinary care team. This role requires excellent customer service skills, a strong clinical background, and demonstrated leadership. The Nurse Liaison plays a critical role in ensuring the timely and seamless transition of infusion patients from hospital to home. Serving as the first point of contact for new home health referrals, the Liaison is responsible for clinical coordination, reviewing documentation to ensure appropriateness and readiness for home infusion services, and facilitating effective communication among care teams.

The Nurse Liaison works closely with physicians, hospital discharge planners, and interdisciplinary team members to develop individualized plans of care. In addition, this role provides education to patients and caregivers, ensuring they feel confident and supported in managing the IV care required s job description integrates the ANA Nursing:
Scope and Standards of Practice, and the ANA Code of Ethics for Nurses with Interpretive Statements, and the UVA Nursing Professional Practice Model.

Continuum Home Health Nurse Manager

MINIMUM REQUIREMENTS:

  • Education:

    Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.
  • Experience:

    1 year of relevant experience.
  • Licensure:
    Licensed to practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver's License required. American Heart Association (AHA) Health Care Provider BLS certification required.

PHYSICAL DEMANDS:

Job may require sitting for prolonged periods, frequently standing, traveling, bending/stooping. Proficient communicative, auditory and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease. The pay range for this role is $82,992.00 - $ annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
