Case Management Rn Hospital Inpatient

Seeking an experienced Case Management RN for a 13-week hospital inpatient assignment. The role involves coordinating patient care across various units, ensuring compliance with hospital policies, and managing patient discharge planning.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Collaborate with multidisciplinary teams to facilitate effective patient management.
• Approve up to 40 hours of RTO during the contract period.
• Ensure compliance with Epic electronic medical records systems and hospital protocols.
• Disclose any scheduled time off before start date and verify all licensing and certifications.
• Document and communicate patient progress and discharge instructions accurately.

Qualifications:

• Hold an active RN license; pending licenses accepted if in hand by Wednesday before start date.
• Possess a minimum of 1 year of acute care hospital inpatient case management experience.
• Exhibit proficiency in Epic EMR systems; travel experience preferred.
• Be able to verify Nursys information upon submission.
• Demonstrate strong clinical assessment and documentation skills.
• Provide current certifications and professional references.
