As a Registered Nurse Care Coordinator, you will
support patients across several cardiology subspecialties
, including Heart Failure, Structural Heart & Valve, Pulmonary Hypertension, and Vascular Medicine. Working alongside physicians, advanced practice providers, medical assistants, and fellow RN Care Coordinators, you will play a critical role in helping patients navigate their care journey while contributing to exceptional clinical outcomes.

The Role

The RN Care Coordinator serves as an essential liaison between patients and providers, helping coordinate care for individuals with complex and chronic cardiovascular conditions. This role blends direct patient interaction with clinical coordination responsibilities, creating a dynamic environment where strong assessment skills, organization, and communication are essential. The position offers a predictable schedule
- Monday through Friday, 8:00 AM to 4:30 PM
, with
no weekends, no holidays, and no call responsibilities
. Hybrid flexibility may also be available based on assignment and operational needs.

In an average day, you may:

  • Perform patient intake and review allergies, medications, social history, family history, and symptoms.
  • Obtain detailed patient histories and complete symptom assessments.
  • Prepare patients for provider visits and procedures.
  • Review referrals and coordinate care plans.
  • Respond to patient calls and Epic In-Basket messages.
  • Process prescription refill requests and support medication management activities.
  • Coordinate diagnostic testing and specialty appointments.
  • Educate patientsregardingdiagnoses, treatments, medication adherence, and symptom monitoring.
  • Support chronic disease management programs and long-term follow-up plans.
  • Collaborate closely with physicians, nurse practitioners, physician assistants, and medical assistants to ensure seamless patient care.
  • Support Heart Failure populations through provider-directed medication adjustments, laboratory follow-up, blood pressure and weight monitoring, and ongoing patient outreach.
