This is an exceptional opportunity for an

Working alongside a highly specialized transplant team, you will play a critical role in ensuring patients receive seamless, patient-centered care from referral through post-transplant follow-up while building expertise in one of the most advanced areas of oncology nursing.
** The Role*
* As the Registered Nurse Care Coordinator, you will coordinate care for adult stem cell transplant and cellular therapy patients across the continuum of care. This role combines clinical expertise, patient advocacy, education, care navigation, and multidisciplinary collaboration in a fast-paced specialty setting where every interaction has meaningful impact.
Success in this position comes from strong organizational skills, exceptional communication, sound clinical judgment, and the ability to build trusted relationships with patients, families, providers, and colleagues.
In an average day, you will:
+ Coordinate care for adult stem cell transplant and cellular therapy patients from referral through post-transplant follow-up.
+ Serve as a primary point of contact for patients and families, providing education, guidance, and support throughout their treatment journey.
+ Manage transplant timelines and ensure critical milestones are completed accurately and efficiently.
+ Collaborate closely with physicians, advanced practice providers, pharmacists, social workers, and other multidisciplinary team members.
+ Facilitate communication across departments to support safe, coordinated, patient-centered care.
+ Help patients navigate complex treatment plans, appointments, and transitions between phases of care.
+ Apply critical thinking and clinical expertise while managing multiple priorities in a dynamic ambulatory environment.
+ Build lasting relationships with patients during an important and often life-changing chapter of their healthcare journey.
** This is a full-time, Monday through Friday clinic-based position with no weekends or holidays. Remote flexibility may be available based on operational needs.*
