Role Overview :
Part-Time Psychiatric Mental Health Opportunity, Charlottesville, VA

We are seeking an experienced and collaborative Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our Charlottesville team on a part-time basis. This role provides high-quality, person-centered psychiatric assessment, diagnosis and treatment within post-acute, long-term care and assisted living settings. The successful candidate will evaluate mental health and behavioral concerns, develop and manage individualized treatment plans, monitor treatment effectiveness, support medication management, coordinate care with interdisciplinary teams and promote safe, effective continuity of care.

Key Responsibilities
  • Provide comprehensive psychiatric mental health assessment, diagnosis and treatment to patients in post-acute, long-term care and assisted living settings
  • Conduct initial, routine and follow-up psychiatric evaluations, including review of mental health history, symptoms, medications, diagnoses and current concerns
  • Assess and manage a range of psychiatric, psychological and behavioral health conditions within the scope of practice
  • Evaluate mood, affect, thought processes, cognition, behavior, functional status, safety risks and changes in clinical presentation
  • Develop, implement and review individualized psychiatric treatment plans in collaboration with patients, families, facility staff and interdisciplinary care teams
  • Prescribe, review and manage psychiatric medications in accordance with applicable regulations, clinical guidelines and patient needs
  • Monitor treatment response, therapeutic outcomes, adverse effects, drug interactions, polypharmacy and clinical risk
  • Assess and manage changes in mental health status, acute psychiatric needs, behavioral concerns and urgent patient issues, escalating care appropriately when required
  • Evaluate patients for suicidal ideation, self-harm, aggression, psychosis, delirium and other safety concerns, initiating appropriate interventions and referrals
  • Order, review and follow up on relevant laboratory tests, diagnostic investigations and other clinical information related to psychiatric treatment
  • Collaborate with physicians, nurse practitioners, nurses, psychologists, therapists, specialists, hospitals and community mental health resources
  • Participate in care planning meetings, interdisciplinary reviews, family consultations and facility clinical meetings as required
  • Support appropriate transitions of care, hospital follow-up and coordination of psychiatric and behavioral health services across the continuum
