Salary Range: $24.79 – $28.00 per hour

Schedule:

Monday – Friday, 8:30 AM – 5:00 PM

Under the general supervision of the Corps Officer or the Social Service Director, the Case Manager implements a strengths‑based approach to client caseloads. The role involves conducting assessments and intake, establishing professional relationships, and helping clients develop and achieve personal, familial, educational, career, housing, financial, and spiritual goals. The Case Manager advocates for clients, facilitates access to services, coordinates community agency resources, and maintains client files in accordance with best practices.

• Conduct client meetings and build collaborative relationships focused on client engagement.
• Advocate for clients by directing them to service providers, explaining choices, and encouraging informed decision‑making.
• Maintain accurate, up‑to‑date client files and case management notes, including narratives, case plans, incident reports, client logs, and related paperwork.
• Initiate contact with community agencies on behalf of clients and coordinate delivery of services through case reviews and team meetings.

Work is performed in a normal office environment with minimal physical discomfort.

• Associate’s Degree in Social Work, Human Services or a related field, and 1‑2 years of experience performing Social Work using a strengths‑based approach.
• Experience providing direct case management services, accessing client needs, and developing comprehensive, long‑term action plans for recovery.
• Any equivalent combination of training and experience that provides the required knowledge, skills and abilities.
