Physical Therapist

OSR Physical Therapy is a local physical therapist-owned private practice established in 2005 with five locations in Minnesota. We provide physical therapy and athletic training outreach to schools, sports clubs, dance studios, and various fitness and wellness services.

It is our goal that all patients reach their optimal recovery  patients’ rehabilitation experience will always be positive by applying exceptional customer service and clinical skills. • Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

• Graduate degree from CAPTE-accredited Physical Therapy program. New graduates are encouraged to apply, we’d love for you to start your career with us!
• Current state of Minnesota license and CPR certification
• Ability to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans or willingness to learn
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office

The salary range begins at $72,000 and increases based on relevant experience.Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for discretionary incentive compensation. The Company's incentive compensation plan is subject to change.
