The Hamilton County Health & Social Services Division has a responsibility to respond quickly and efficiently when public health emergencies occur. All Hamilton County Health & Social Services Division employees are responsible for ensuring their contact information is current and they are accessible should a public health emergency arise. The Hamilton County Health & Social Services Division maintains a call down system to store contact information and mobilize staff in the event of an emergency.
During a public health emergency, all staff are required to respond should a call to mobilize be made. During a public health emergency, work hours for any and all Hamilton County Health & Social Services Division employees are subject to modification to meet the needs of the community.
Under general supervision, responsible for assisting in providing coaching, onboarding, training, and reflective supervision for the CHANT staff. Responsible for quality assurance and compliance with program regulations and procedures which includes monitoring pathway progression of families on the CHANT and CYSHCN programs. Responsible for coordinating and planning targeted community–based outreach activities. Assists the Health Program Manager with troubleshooting programmatic and staffing concerns and is presently available for the staff on a regular basis.
ESSENTIALFUNCTIONS
Responsible for providing daily assistance to Care Coordinators as needed with pathway progression either in daily consultation or via reflective supervision. Works closely with the Metro Lead (Health Program Manager) Medical Services Lead (Nurse Specialist) Team Lead (Social Worker II), and Medical Services assistants (Health Case Managers) to streamline any internal process to assist Care Coordinators with the CYSHCN application process.
Ensures staff has equipment and necessary tools to perform daily CHANT duties (i.e., tablet, phone, Bluetooth, monitors, etc.).
Responsible for organizing and implementing the monthly, targeted based community outreach engagement event as needed. Assigns and supervises / monitors team participation for community-based events. Responsible for updating the CHANT community outreach report in Red Cap on a regular basis and identifies, establishes, and maintains relationships with community partners to engage collaboration.
Coordinates & assists with leading CHANT huddles (team meetings) in a team leadership approach.
Monitors and conducts daily supervision of progression of CHANT staff along with training and onboarding. Follows up with Care Coordinators on performance via desk audit and reflective supervision.
Provides Care Coordination services as needed in the event of vacancies and/or limited staff.
Works in tandem with the Metro Lead in the development and implementation of CHANT in accordance to programmatic guidelines.
Serves as a communication “link” (along with other leads) between Central Office and CHANT staff.
Provides reflective supervision, coaching, and training for Care Coordinators per guidelines. Attends and conducts home visits as needed per Care Coordinator request and for observation/evaluation purposes and reflective supervision purposes per guidelines. Assists in seeking out appropriate staff trainings and development and keeps a log of each Care Coordinator’s progression. Reviews data reports for continuous quality improvement (monthly along with other team leads).
Assists in preparing quarterly reports for CHANT compliance with Central Office and attends in-person or virtual quarterly meetings.
Monitors program for compliance with contractual obligation per grant allocation. Identifies, engages, and maintains relationships with community partners. Attends trainings, conferences, conference calls, and meetings required by the CHANT state grant. Adheres to Hamilton County HR policy and evaluation processes for supervised staff under the supervision of the Health Program Manager.
