What to Expect (Job Responsibilities)
  • Partner with members and families to identify necessary supports and services to achieve personal health and wellness goals
  • Collaborate with a team of clinical and social support professionals to address members' physical, behavioral health, and long-term service needs
  • Conduct thorough face-to-face assessments to evaluate each member's current status and needs
  • Authorize appropriate home and community-based services, ensuring they are provided effectively
  • Maintain a valid driver's license and comply with safety program requirements for vehicle operation
What is Required (Qualifications)
  • 3 years of experience in a clinical setting
  • Registered nurse with an active license in the state of Tennessee or a participating state in the Nurse Licensure Compact Act; or a Master’s in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW)
  • Minimum of 2 years of clinical experience required
  • Proficiency in basic Microsoft Office and email applications
  • Excellent oral and written communication skills, along with strong interpersonal and organizational skills

  • Experience in case management or care coordination within a healthcare setting
  • Familiarity with community resources and services available to members
  • Advanced certification in nursing or social work specialties
  • Bilingual proficiency in relevant languages
