Job Summary

The Nurse Practitioner or Physician Assistant will provide high-quality primary care and support services to individuals and families within the local community. This position is a full-time, day‑shift role, Monday through Friday, from 8:00 a.m. to 5:00 p.m. The provider will work collaboratively with physicians, nurses, and other healthcare professionals to deliver patient‑centered care in a community health setting.

Responsibilities include conducting comprehensive health assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans. The NP/PA will provide preventive care services such as immunizations, screenings, and health education to promote wellness and disease prevention. This role also involves managing follow‑up care, coordinating referrals to specialists when necessary, and ensuring continuity of care for patients.

The provider will maintain accurate and timely documentation in electronic health records, adhere to clinical protocols and regulatory requirements, and participate in quality improvement initiatives. Strong communication skills are essential for educating patients about their health, collaborating with interdisciplinary teams, and fostering trust within the community. The NP/PA will also play a key role in addressing social determinants of health by connecting patients to appropriate resources and support services.

This position requires a commitment to compassionate, culturally competent care and the ability to work effectively in a fast‑paced environment while prioritizing patient needs.

This position is involved in a team‑based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. The provider is trained and assigned (as per their role and responsibilities) to coordinate care for patients, support patients/families/caregivers in self‑management, self‑efficacy and behavior change, and manage the practice's patient population.

The provider participates in the practice's quality improvement process and performance evaluation.

Education
  • RN with a master's degree in nursing or certification in family and/or adult nurse practitioner.
  • Graduate of a nationally accredited school of nursing.
Experience
  • Previous experience in a FQHC – strongly preferred.
  • Experience with evidence‑based protocols and utilizing them to develop an individualized plan of care.
  • Experience in developing and implementing health screening programs for adolescents and adults, and developing protocols for identification of high‑risk individuals.
  • Experience in community and corporate education on internal medicine topics.
Position Requirements :
License/Certification/Registration
  • Current licensure to practice nursing in the State of Tennessee.
  • Eligibility to sit for certification in area of specialty.
  • Current licensure or multistate licensure from a compact state must be obtained within three months of hire for non‑Tennessee residents.
Preferred
  • Certification in areas of specialty.
Department Position Summary

A nurse practitioner in PH Family Practice/Internal Medicine will work under direction of the physicians/medical director. The provider will be responsible for developing evidence-based protocols and utilizing them to develop an individualized plan of care, collecting, documenting, and analyzing patient data, and collaborating with the health care team in the development and implementation of testing and treatment according to protocols. The provider will participate in the development and implementation of a health screening program for adolescents and adults and develop protocols for identification of high‑risk individuals.

The provider will also participate in community and corporate education on internal medicine topics.

The individual must demonstrate knowledge of the principles of growth and development over their life span and possess the ability to assess data reflective of the patient's status. The provider must interpret the appropriate information needed to identify each patient's requirements relative to his or her age‑specific needs and provide the care needed as described in the unit's policies and procedures.

Competency is evaluated by direct observation or a formal peer review process or self‑assessment based on objective criteria that is reviewed and validated.
