Position
LPN, Licensed Practical Nurse PRN
Pay$20.00 - $35.00 hourly, depending on experience and responsibilities.
DescriptionCaris Healthcare’s Hospice Licensed Practical Nurse (LPN) provides quality patient care and performs technical skilled care in patients’ homes. This position assists home health aides in fulfilling their responsibilities.

- Provides technical care to patients, receives assignments, reports, and provides prescribed medication under supervision of RN
- Observes, records, and reports all observations of the patient's mental and physical care provided
- Performs special procedures/treatments for which they have been instructed and supervised by an RN
- Notifies the Patient Care Coordinator and/or physician immediately of any changes in patient's condition; documents changes in the clinical record
- Meets periodically with the Patient Care Coordinator to discuss nursing care, policies, and future planning
- Functions as a link to help the agency comply with regulatory requirements
- Participates as an IDT team member, formulating the interdisciplinary care plan with other IDT members
- When requested, assists home health aides in performing their duties and assists the physician or RN with specialized procedures
- Willing and able to accurately utilize automated devices required to document into the electronic medical record
- Participates in on‑call according to rotating schedule
- Must be licensed in the state of operation
- Must have at least one year of nursing experience and complete knowledge of technical skills required for nursing procedures
- Possess an understanding of hospice care concepts and the role as a member of an interdisciplinary care team
