Full Time, 48 Hours Per Pay Period, Variable Shifts. 8am-8pm on Saturday and Sunday. On-call required for 8pm-8am shifts on each day.
OverviewCovenant Health Home Care and Hospice is the area’s largest locally owned home health and hospice provider. The position provides professional nursing care to terminally ill patients during weekends.
Position SummaryProvides professional nursing care and coordinates care planning with the interdisciplinary team to ensure the best outcomes for patients’ remaining time. The RN‑Hospice works 12‑hour weekend shifts and two 12‑hour on‑call shifts starting Saturday 8AM and Sunday 8AM. Reports to the assigned Hospice Clinical Supervisor.
Responsibilities- Conduct initial and ongoing assessment of the patient's physical, functional, psychosocial, and environmental needs.
- Implement individualized plan of care and recommend revisions as necessary.
- Facilitate palliative milieu and manage symptoms focusing on comfort at end of life.
- Consult and educate patients/families about the disease process, self‑care techniques, end‑of‑life care and ethical issues.
- Initiate preventive and rehabilitative nursing procedures (pressure ulcer prevention, pain crisis prevention, bowel impaction prevention, care of bed‑confined patient).
- Prepare clinical and nursing notes reflecting disease progression.
- Coordinate patient/family services and prioritize needs with interdisciplinary team.
- Use case‑management approach and refer to other services as needed.
- Serve as liaison between providers, patients/families, and staff.
- Determine scope and frequency of services and adjust visit calendar as needed.
- Supervise LPNs and Hospice/Home Health Aides; orient new employees.
- Provide specialized hospice training to staff, family members, and informal caregivers.
- Evaluate patient/family response to education and care.
- Advocate for respite care and assess caregiver ability.
- Maintain knowledge of Medicare guidelines and comply with policies.
- Adhere to admission, discharge, on‑call, and death procedures as per Covenant Home Care policy.
- Perform basic infection control techniques per policy.
- Preplan visits with patient/caregiver ETA to home.
- Demonstrate competency in age‑specific, culture‑specific, and religious‑specific care.
- Complete required education assignments annually and participate in quality improvement initiatives.
- Perform other duties as assigned.
- Graduation from an accredited school of nursing.
- Minimum 3 years of professional experience in hospital or prior home health, with clinical experience in hospice, oncology, or palliative care preferred.
- Valid Tennessee RN licensure and CPR certification.
- Valid Tennessee driver’s license Class D and minimum insurance coverage required. Must maintain a driving record that meets Covenant Health standards.
