Position Overview
We have an exciting opportunity for you to join Parkridge Medical Center, part of the nation's leading provider of healthcare services, HCA Healthcare, as a Registered Nurse in Admission and Discharge.
Job Summary and QualificationsAs a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient‑centered care. The RN is dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute‑care hospital settings, providing specialized assessment skills to place patients in the appropriate care areas and facilitate discharge. The role focuses on advocating for patients, families, and caregivers while embodying the organization’s vision, mission, and values, aiming to provide an unmatched patient experience and optimal clinical outcomes.
Responsibilities- Obtain nursing assessment history and ensure proper inpatient care unit admission; take appropriate action when needed, such as communicating special circumstances to physicians, care teams, or reporting abuse or neglect.
- Complete and facilitate required documentation, including admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post‑hospital documentation.
- Teach patients, families, and caregivers about the patient’s medical condition, status, treatment plan, medications, possible side effects, and follow‑up measures, translating complex medical terminology to ensure complete understanding.
- Complete additional discharge documentation, education, and discharge care for special patient populations to ensure compliance with core measures for discharge.
- Ensure patients, families, and caregivers receive copies of discharge paperwork and any other required documentation, such as referrals for ongoing care and educational materials.
- Coordinate with House Supervisors and Nursing Administration to prioritize workload and identify areas of greatest need, ensuring smooth patient transitions.
- Demonstrate time‑conscious awareness and ability to prioritize, distinguishing between low‑ and high‑priority activities.
- Advanced Cardiac Life Support must be obtained within 60 months of employment start date.
- Basic Cardiac Life Support must be obtained within 30 days of employment start date.
- Registered Nurse license.
- Associate Degree, Registered Nurse Diploma, or Bachelor’s Degree.
