LPN (Licensed Practical Nurse)

Salary and Sign‑On: Up to $35 per hour. $3,000 sign‑on bonus available.

Shifts: Day, Night, Full time, Part time, Per Diem.

• Medication administration and treatment.
• Assist with admission, transfer, and discharge of residents.
• Document resident care services by charting in resident medical records and department records.
• Educate residents and families regarding treatment, rehabilitation, and changes in status.
• Adhere to infection control policies and procedures and all facility policies.
• Consult with residents’ physicians regarding plan of care and notify them of any changes.
• Maintain residents’ confidentiality and privacy.
• Ensure quality of care by adhering to DOH standards of practice and facility standards.
• Collaborate with the interdisciplinary team to solve residents’ problems.

• Possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
